     Insurance Reimbursement
    
Due to the high cost of processing insurance claims, SMI does not accept health insurance. We require payment at the
     time of your appointment, BUT we will provide the necessary information and do everything we can to help you obtain
     reimbursement from your insurance company. You must have a
PPO or similar insurance plan to be eligible for
     reimbursement. You should also see your
doctor (MD, Chiropractor, or Osteopath) and have him/her provide you with
     a
diagnosis and icd-9 code (a number that corresponds to the diagnosis). We do have a chiropractor on staff who
     can diagnose your injury.

     We will provide you with a
Superbill for each appointment that will contain all the required information your insurance
     company will need. We will also provide a letter to be signed by your doctor indicating the necessity to receive treatment
     at SMI. Patients who have PPO’s and similar plans regularly receive
60–100% coverage for our services.

     Flexible Spending Plan/Health Savings Account
     You can also use your Flexible Spending Plan or Health Saving Account to pay for our services. We will provide you with
     a Superbill for you to submit for reimbursement. You may need a recommendation from your doctor in order to access
     funds in your HSA or Flex Spending Plan. We will provide you with a letter your doctor can sign which indicates treatment
     is recommended at SMI.


     We DO NOT accept any worker’s compensation cases.
